MD Refresh is located at 1522 Makaloa Street, Suite 201. Drive down Keeaumoku St. towards Ala Moana, you turn left when you see the new Walgreens. The second building on the left is a 2 story beige building. This is Makaloa Plaza. The sign, MD Refresh is above Pandora Cafe. The entrance to the parking lot, which is behind the building, is on the Diamond Head side of the building.
MD Refresh Office Policies
Welcome to M.D. Refresh! We hope you enjoy your time in our office. We will strive to help you make you look and feel your best.
To avoid any misunderstandings, here we give you full disclosure of our office policies:
Initial Consultation: Yes, we do offer all patients a free initial consultation. Please arrive 15 minutes before your first scheduled appointment so that you may complete the new patient forms. If you are late, we may not be able to give you the complete initial consultation as we have an obligation to see our next patients in a timely manner.
Payments: Payments may be made by Visa, MasterCard, or cash. We do not accept Amex or checks. Please note that prices are subject to change without notice. Although your initial consultation is free, you must leave a valid Visa or MasterCard number with us to guarantee your next appointment. However, your card will not be charged until the time of your next appointment.
Cancellations and missed appointments: We allot a minimum of 30 minutes for every appointment, so if you miss your appointment or cancel at the last minute, we will not be able to fill in that appointment. To avoid a cancellation penalty, you must cancel by texting us at 808-469-1997 greater than 48 hours before the appointment. Failure to adhere to this policy will result in a $100 charge to your credit card. This $100 charge is much less than the penalty other offices charge, which is between 50% and 100% of the full cost of a missed procedure.
Refunds and retreatments: Once we provide you a service, we do not offer any refunds or free/discounted retreatments for any reason, even if you are disappointed or unhappy with the results.
Product Refund Policy: We do not offer any refunds on products purchased. If you have a documented allergic reaction to a non-prescription product within 7 days of purchasing it, we will give you an in store credit only. In accordance with federal law, no refunds or exchanges can be given for prescription medications for any reason. If you have a defective product, such as a broken dispenser, within 7 days of purchase, we will make an exchange for the exact same product only.
